Abolish the gender clinics. Move it into primary care, specified in the GP contract (or abolish the whole private GP system and bring it into the NHS). Move to informed consent, GPs have sufficient training for HRT and monitoring, they just choose not to.

The whole private GP system sucks anyway. 1 problem per visit, 10 minute visit max. Like a lot of health issues can be distilled down to a 10 minute visit? Telling people to limit it to one problem forces them to figure out what symptoms are relevant and obliterates thue wholistic diagnosis.
