You need two things for a succesfull lethal injection: 1) A quaified doctor or nurse who knows how to insert an IV 2) High quality drugs provided by a reliable manufacturer. Neither doctors / nurses nor drug manufacturers are willing to provide these for capital punishment.

As a result, you have amateurs injecting degraded black market drugs into the inmate's body, often missing the vein.
