Can someone explain to me like I'm five how this happens? In countries where legal, I can book an assisted death and guarantee success 100 out of 100 times. And also, I'm absolutely aware that the death penalty has zero benefits and costs more than life in prison.

You need two things for a succesfull lethal injection: 1) A quaified doctor or nurse who knows how to insert an IV 2) High quality drugs provided by a reliable manufacturer. Neither doctors / nurses nor drug manufacturers are willing to provide these for capital punishment.

As a result, you have amateurs injecting degraded black market drugs into the inmate's body, often missing the vein.
