Then why, when I worked in the USA did I get several patients asking if I was from Canada? And how is it that after living there for a number of years I could hear distinct differences in how Canadians pronounce certain words?

If we just want a few states, I would start with all five states that touch the Pacific Ocean, Puerto Rico, and the USVI. USA does not want Puerto Rico or USVI anyway, so they would give them away for a handful of beads and $24. A precedent was set in 1626.
